Tackling TMJ Pain: When Surgery Might Be the Right Choice

Pain in your jaw, headaches, difficulty chewing, or a persistent clicking sound when opening your mouth—these are just a few signs of temporomandibular joint (TMJ) disorder. TMJ issues can significantly impact your quality of life, and while many patients find relief through conservative therapies, some cases may require a more advanced solution. At Central Valley Dental Implant and Oral Surgery Institute, we help patients throughout the Central Valley find lasting relief—including when TMJ surgery may be the right choice.

Understanding TMJ Disorder

The temporomandibular joint acts as a hinge connecting your jawbone to your skull. This complex joint allows you to speak, chew, and move your jaw. TMJ disorders can arise from:

  • Jaw injury or trauma
  • Arthritis
  • Bite misalignment (malocclusion)
  • Bruxism (teeth grinding or clenching)
  • Disc displacement within the joint

Symptoms can range from mild discomfort to chronic pain that disrupts daily life.

Non-Surgical Treatments First

At CVDI, our team of oral and maxillofacial surgeons—Dr. Bell, Dr. Rahn, Dr. Smith, Dr. Anderson, Dr. Bunnell, and Dr. Henriksen—prioritize conservative treatment before recommending surgery. Common non-invasive options include:

  • Physical therapy and jaw exercises
  • Custom night guards to prevent grinding
  • Anti-inflammatory medications
  • Stress reduction and posture correction
  • Injectables, such as corticosteroids or Botox

For many patients, these methods offer significant relief. But when TMJ pain persists or worsens despite treatment, it may be time to explore surgical solutions.

When Is TMJ Surgery the Right Choice?

TMJ surgery is typically considered for patients who:

  • Have severe, chronic jaw pain unresponsive to non-surgical treatment
  • Experience locking of the jaw or inability to open/close the mouth properly
  • Show evidence of joint damage, degeneration, or disc displacement
  • Have structural abnormalities confirmed through imaging studies (MRI or CT scans)

A detailed consultation and imaging evaluation at CVDI helps us determine whether surgery is appropriate—and which type of procedure offers the best outcome.

Types of TMJ Surgery

  1. Arthrocentesis

A minimally invasive procedure where the joint is flushed with fluid to remove debris and reduce inflammation. Often used for patients with mild to moderate symptoms.

  1. Arthroscopy

Using a tiny camera and small instruments inserted into the joint space, surgeons can diagnose and treat internal joint issues with minimal trauma and a faster recovery.

  1. Open Joint Surgery (Arthrotomy)

For more complex conditions such as severe arthritis, scarring, or abnormal growths, open surgery may be needed to repair or remove damaged tissue.

  1. Joint Replacement

In rare, advanced cases where the joint is severely degenerated, total joint replacement may be necessary. This is typically considered a last resort and is performed by highly specialized oral surgeons.

What to Expect During the Process

Our surgeons will guide you through every step:

  • Comprehensive evaluation with imaging
  • Thorough explanation of your

     condition and treatment options

  • Personalized surgical plan
  • Post-operative care instructions for a smooth recovery

Recovery time varies by procedure, but most patients experience significant improvement in jaw function and pain relief after surgical treatment.
